He explains that being booked out too far, more than two weeks is a big problem because it increases the likelihood of no-shows and cancellations, which reduces profitability. 

To maximize profitability, practices should aim to be booked out no more than 10.5 business days. If a practice is booked out too far, it can either expand capacity or cut demand. Cutting demand involves dropping low-paying insurance plans and raising fees to reduce the number of patients seeking treatment, allowing the practice to charge higher fees and increase profitability. 

Dr. Scott Leune provides an example of a dentist who went from a take-home pay of $350,000 to $1.1 million by dropping all insurance plans and going out of network with everything.
